2. Parties: City of Port St. Lucie and Life Scan Wellness Center.
3. Purpose: To offer a value-added wellness benefit to the City of Port St. Lucie employees.
4. New/Renewal/Modified: Renewal.
5. Duration: Initial Term: August 27, 2019 through August 26, 2022. The City of Clermont exercised two (2) one-year renewals through August 25, 2024. Current one (1) year renewal: August 26, 2024 through August 27, 2025, requested in Amendment #4. One (1) additional one-year renewal remains.
6. Benefits to Port St. Lucie: Reduced medical claims by providing a focus on early detection and treatment through an advanced level of testing and patient assessment.
7. Cost to Port St. Lucie (Annual and Potential): Initial Annual Cost: $135,000.00. The total amounts for the Amendments are as follows: #1 - No cost change, #2 - $8,100.00, #3 $8,900.00, #4 - $35,000. Total annual estimated cost: $187,000.00. Future expenditures will be appropriated as each year's budget allows.

Staff Recommendation: Move that the Council approve the proposed Contract Amendment #4 and ratify Amendments #1-3.

1. Move that the Council amend the recommendation and approve Contract Amendments #1-4.
2. Move that the Council not approve and provide staff with direction.
Background: The Life Scan Wellness program is an integrated medical approach that combines an annual physical with a focus on lifesaving early detection testing for major diseases, such as heart disease, stroke, cancer, diabetes, and aneurysms before they reach a catastrophic level. The Life Scan evaluation provides employees with a thorough assessment of their health as well as providing recommendations for achieving and maintaining long-term wellbeing and management of health risks. Life Scan offers on-site, same day examinations at the City Hall Complex. Life Scan's successful early detection rates reduce health care costs and workers compensation costs because many health issues are identified before the reach catastrophic levels. Plus, the Life Scan program has a lower overall cost than many medical providers offering routine occupational physicals.
Amendment #1: Contract Renewal for a one-year term 8/27/22 through 8/26/2023.
Amendment #2: Amount Increase - $8,100.00 for a total of $143,100.00.
Amendment #3: Contract Renewal for a one-year term 8/27/23 through 8/26/2024.
Amount Increase - $8,900.00 for a total of $152,000.00.
Amendment #4: Contract Renewal for a one-year term 08/27/2024 through 08/26/2025.
Amount Increase - $35,000 for a total of $187,000.00.
Issues/Analysis: Staff has reviewed the additional compensation and find that it is fair and reasonable for the additional services to be provided.
Financial Information: Funds are appropriated in MED TRUST / GENERAL GOVERNMENT / PROFESSIONAL SERVICES (605-1900-531000-00000).
