2. Parties: City of Port St. Lucie and PSL Medical Provider, Inc. aka Treasure Coast Medical Associates, Inc. (TCMA).
3. Purpose: To provide medical services through the On-Site Employee Medical Clinic for employees, dependents and retirees.
4. New/Renewal/Modified: New.
5. Duration: Five (5) years with an option for an additional five (5) year renewal. Effective October 1, 2019 through September 30, 2024.
6. Benefits to Port St. Lucie: The contract provides necessary primary, urgent, acute, episodic, wellness and other health screening services to the City employees, dependents, and retirees as needed.
7. Cost to Port St. Lucie (Annual and Potential): Projected annual expenditures will be $1,563,640.
